To ensure that the lessons of Averil's tragedy are learned and change implemented widely.
To gain a sincere and appropriate apology from the NHS to Averil, her family, her boyfriend, friends and relatives.
To encourage changes in the focus of NHS complaints handling from the service itself to a coherent patient- and family-focused procedure, which spans multiple organisations, and has openness at its core.
